Abstract
A case of astemizole-induced QT prolongation, T-wave alternans, and torsades de pointes is presented. Failed treatment with intravenous magnesium and lidocaine was followed by overdrive pacing, which successfully prevented further recurrences. The possible pathophysiology of acquired QT syndrome and T-wave alternans after antihistamine use is discussed.
