Recurrent encephalopathy in hyperparathyroidism

Abstract

An unusual clinical presentation of intermittent episodes of organic brain syndrome with lucid intervals in a hypertensive patient is reported. Blood calcium measured during the first attacks was between 11.0-12.0 mg%. In several attacks there was lethargy with disorientation, hallucinations, coarse tremor and pyramidal tract signs associated with 9.8-10.7 mg% serum calcium. Cerebrospinal fluid calcium was found to be elevated twice. All neurological and psychotic signs disappeared following excision of a 5 mm hyperplastic parathyroid nodule. Calcium levels decreased and the EEG pattern, which was abnormal during the attacks as well as during the intervals, became normal after the operation.
