note = "Funding Information: At the outset it seemed to us that the task of providing a comprehensive reference source divided into survey chapters that summarize frontier areas of research, elucidate new theoretical developments, and review the existing evidence, would be made considerably easier if we could bring together the authors of the Handbook chapters at the stage at which detailed chapter outlines were in hand. Such a meeting would acquaint all authors with each other's planned work, assist in filling gaps, help achieve appropriate coverage, identify interconnections, and limit unwarranted duplications. We are indebted to the Ford Foundation and its Reproductive Health and Population Division for providing us, through a grant to Harvard University and the University of Pennsylvania, with financial support for our authors' conference. We believe that the conference was highly conducive to transforming the Handbook into a product more valuable than the sum of its parts.
