Abstract

To the Editor: In his article on nitrate therapy in stable angina pectoris (June 25 issue),1 Parker stated that “nitrate tolerance is an important problem” in maintenance nitrate therapy but failed to acknowledge the accompanying risks of nitrate habituation. The occupational-medicine literature of the past 40 years documents the consistent international occurrence, primarily in the explosives industry, of clinical coronary pain and sometimes death in workers habituated to the organic aliphatic nitrates.2 3 4 These untoward events typically occur on vacations or toward the end of a weekend, as though the coronary vessels were reacting to the withdrawal of the nitrates.
