Abstract

A 90-year-old man was admitted because of severe epigastric pain, fever cyanosis and dyspnea. X-ray revealed left pleural effusion which was found to contain 24000 untis (Phadebas) of diastase, the key for the correct diagnosis of spontaneous perforation of the esophagus. It is recommended that diastase be measured in every case of pleural effusion when the underlying disease is not known.
