Association between mitral annulus calcification and anti β2- Glycoprotein I antibody levels

Abstract

Background. Mitral annulus calcification (MAC) has been reported to be associated with carotid artery atherosclerotic disease, coronary artery disease, and aortic atheroma. Antibodies to cardiolipin, β2-Glycoprotein I (β2GPI), and oxidized low density lipoprotein (LDL) have recently been associated with atherosclerosis. In the present study we aimed at determining the possible association of MAC with each of these autoimmune parameters. Methods. 74 patients who underwent coronary angiography because of anginal syndrome were studied for possible association between coronary atherosclerosis and several autoimmune markers. Of the patients, 50 underwent transthoracic echocardiography (TTE) in our laboratory before the procedure and were the subject of the present study. MAC was defined as a dense localized, highly reflective area at the base of the posterior mitral leaflet in TTE and was considered severe when the thickness of the localized highly reflective area was ≥ 5 mm on 2-D echocardiography in the four chamber view. Antibodies to cardiolipin, β2GPI, and oxidized LDL were determined using enzyme linked immunosorbent assay (ELISA). Cutoff for anti β2GPI antibody positivity was determined as 25 GPL units. Results. Patients with severe MAC (≥ 5 mm) were more likely to be positive for anti β2GPI antibodies when compared with patients without MAC (56% vs. 22%, p<0.05). Patients with severe MAC were also more likely to be positive for anti β2GPI antibodies when compared with patients with nonsevere MAC (56% vs. 0% p=0.038). Among the 14 patients with MAC (≥3 mm), a significant correlation was found between anti β2GPI antibody levels and MAC severity (R=0.608, p=0.021). Conclusions. Anti β2GPI antibody levels were strongly associated with the presence and severity of MAC. Should these results be confirmed in larger scale studies, it may be justified to investigate a cause and effect relationship between anti β2GPI autoantibody production and MAC.
